Request Repeat Contact Lenses

Please complete the form below if you are an existing patient and need replacement contact lenses.

Contact lenses can only be ordered when a prescription is up to date and there has been a recent examination of the eyes and lenses. You will be contacted if this is not the case.

You will be notified when to the lenses are ready. Lenses cannot be released without payment and/or completed relevant paperwork (for hospital patients).
